The 31st Annual AIHA Annual Conference, held in Philadelphia, PA hosted healthcare auditors from around the nation. This year’s theme was “Independence and Beyond: Assurance, Insight and Value.” Denise Cicchella, President of Auspicium was pleased to present in the Specialty track and taught auditors best practices in Transition Management of new and renovated healthcare facilities. Previously working with several large hospitals, Auspicium has helped clients assess risk, develop plans, and analyze plans associated with moving patients, staff, equipment, machinery, medications and other items from one facility to another.

Even if the new facility is across the street, transition management of healthcare facilities involves complex maneuvers that need to be carefully planned, coordinated and communicated. Reliance is placed on employees, vendors, patients, patient families, the community and medical specialists working together to ensure a smooth transition and the safety of everyone and everything involved. Coupled with that is the fact that there are a lot of unknowns and unplanned “events” that can change plans at a moment’s notice; for example, patient population and census changes daily.

Identifying and knowing the risks are just as important as being able to control them. Transition management involves more then just moving patients from one site to another. It involves training staff on new facilities, protecting patients’ rights, maintaining patient safety and ensuring the protection of assets as they go from old facility to moving vendor to new facility.

Attendees had the opportunity to learn about the basic risks and plans associated with transition management and to learn first hand about real life examples of problems others ran into during this delicate process and phase.    The presentation was well received by attendees , who felt they received valuable information that they could implicate at their own workplace and during their next transition.
